You can have mental illness AND be a butthole at the same time. Essentially we don't even know what's going exactly we can only speculate. Much in the manner you could start to claim that "the game paints a bad picture of people with a limp because the bad guy has a limp". The entire idea is simple sometimes circumstances can drive a person whose morality was already questionable quite nuts. And I see nothing wrong with having a bad guy be "nuts". Perhaps I'm a mere part of a minority, but I actually do suffer from real mental illnesses and I have experienced psychoses as well. But if the devs are telling a story through that perspective then yes, I do think it is needed and I wouldn't want the devs to refrain from showing the story to us in their way!

What if the devs are trying to present to you a story they made in the form of a game? And if the main character of that story is actually mad and insane, then it's in their right to let the players know that, am I not correct? Even though it's not needed according to the players, what if it is needed for the story according to the devs?
